We're happy you're considering booking an appointment with us! Please fill out the form provided  to request an appointment and will do our best to respond in one business day. If it's your first time visiting us, there's a form we need you to fill out. Here's how you can do it:

1. Download the Form: Click here to download the form to your computer. Fill it out and save it as a new file. Then, email it to us at farmingtonvillagedental[at]gmail.com (remember to use @ instead of [at]). This will speed up your paperwork process.
2. Print and Send: Alternatively, you can print the form, fill it in, and either scan or take a photo of it. Then, email the scanned copy or photo to us.
